iCarePathways
In the Netherlands healthcare professionals (caregivers and nurses) working in the elderly care are struggling since the introduction of the so-called Packages of Care Weights (care funding) with the question how they can offer each patient exactly the right amount and type of care appropriate to the situation and needs of the patient and based on the patient’s indication decision. One of the care organizations involved in this project is Noorderbreedte www.noorderbreedte.eu). Noorderbreedte has developed an electronic care plan (ECP) in which based on the CDS outcomes care arrangements with patients are made to suit their individual needs within the borders of their indication decision (see demo www.ezlp.nl/home_en). This web application establishes a relationship between care professional and patient based on trust to reduce bureaucracy and to start from simplicity.
The CDS forms the basis of this web application: the ECP. The CDS in digital form creates the opportunity for healthcare professionals to visualize care needs and the extent to which the patient is dependent on others. The screen shows also a menu of activities that the care professional can offer the patient. Further, based on the CDS sum score the possible risk of each patient to one or more of the following healthcare problems: pressure ulcer, malnutrition, incontinence, and fall risk, are specified.
Aim of the iCarePathway project is to introduce and test the ECP in other countries e.g. Austria, Germany, UK and to develop care pathways related to the earlier theree mentioned health care problems. Further to implement the ECP and the three care pathways effectively in various health care settings
